    • Work Permit Application 14-17 yr olds
    • RETURNING REFEREES DO NOT NEED TO COMPLETE IF WORK PERMIT ON FILE NEW EMPLOYEE who are 14 - 17 years old, you must obtain a work permit. - Complete the form and get the required signatures. - Bring the form to King Philip High School(main office - summer hours 8:00am - 1:00pm) or if you do not attend KPHS you can go to KPHS or where attend school. - Bring proof of age, such as a birth certificate, passport, or immigration record. - Physician signature or current health form required for 14 and 15 year old only - Receive your Employment Permit. - Give the signed Work Permit to Wrentham Recreation.

    • Federal W-9 Form
    • Tax identification number request form to be completed by individuals, freelancers, independent contractors and consultants who perform work for Wrentham Recreation Department.
