Flag Football Referee 2025

Flag Football Referee Payroll Forms and Instructions All payroll forms must be submitted in person by September 3, 2025
(1) Work Permit Application 14-17 yr olds RETURNING REFEREES DO NOT NEED TO COMPLETE IF WORK PERMIT ON FILE NEW EMPLOYEE who are 14 - 17 years old, you must obtain a work permit from King Philip High School. - Complete form and bring to King Philip High School MAIN OFFICE (summer hours 8:00am - 1:00pm) - Physician signature or current health form required for 14 and 15 year old only - Signed and completed work permit must be turned in with all other payroll forms
(2) Authorization to Release Information Form (PDF) Required for new and returning referees
(3) Direct Deposit Form (PDF) Required for all NEW REFEREES RETURNING REFEREES - only needed if there is a change in your banking information Voided check or bank letter with account number and routing number must be included with this form.
(4) Employee Information or Emergency Contact Form (PDF) Required for new and returning referees
(5) OBRA (VOYA) Enrollment Form (PDF) Required for all new and returning referees Make sure to complete page 1 and 2
(6) I-9 Employment Eligibility Verification Form (PDF) NEW REFEREES must submit this form Complete page 1 Must Present Acceptable Forms of ID (see page 2) RETURNING REFEREES Only needs to complete Supplement B Must Present Acceptable Form of ID (see page 2)
(7) M-4 Massachusetts State Withholding Tax Form (PDF) Required for new and returning referees Check box D (full time student exempt)
(8) W-4 - Federal Withholding Tax Form (PDF) Required for new and returning referees Write "Exempt" in the space below Step 4(c) for full-time student exemption
(9) CORI - Flag Football Referees Required for new and returning referees
