Flag Football Referee 2025

  • All payroll forms must be submitted in person by September 3, 2025
    
    
• • (1) Work Permit Application 14-17 yr olds
  • RETURNING REFEREES DO NOT NEED TO COMPLETE IF WORK PERMIT ON FILE NEW EMPLOYEE who are 14 - 17 years old, you must obtain a work permit from King Philip High School. - Complete form and bring to King Philip High School MAIN OFFICE (summer hours 8:00am - 1:00pm) - Physician signature or current health form required for 14 and 15 year old only - Signed and completed work permit must be turned in with all other payroll forms

  • Required for all NEW REFEREES RETURNING REFEREES - only needed if there is a change in your banking information Voided check or bank letter with account number and routing number must be included with this form.
